ReliaTrain is offering organizations and individuals the opportunity to establish proven knowledge of the engineering core tools for quality and reliability risk assessment. A design organization may either focus on Design Failure Modes and Effects Analysis (DFMEA) or Design Review Based on Failure Modes (DRBFM). A manufacturing and assembly organization would focus on Process Failure Modes and Effects Analysis (PFMEA).

ReliaTrain Certifies 4 Levels of Design Failure Modes and Effects Analysis (DFMEA), Design Review Based on Failure Modes (DRBFM), and Process Failure Modes and Effects Analysis (PFMEA). ReliaTrain can tailor the criteria to meet your organizations unique product and processes.

  • Process Awareness- Ability and knowledge to participate in completing the FMEA Process
  • Competency- Ability and knowledge to lead a team in the FMEA Process
  • Proficient- Ability to teach and mentor Participants and Team Leads in the FMEA Process

Level One: Awareness:

Upon completion of Level One accreditation the participant will be able to:

  • List Planning criteria based on organization and Industry Standards
  • List Preparation criteria based on organization and Industry Standards
  • List Risk Assessment technique based on organization and Industry Standards
  • List Risk Ranking techniques based on organization and Industry Standards
  • List Actions definition and Actions closure techniques based on organization and Industry Standards
  • List Communication and Feedback Loop techniques based on organization and Industry Standards

Level Two: Competency

Upon completion of Level Two accreditation the participant will have completed the following:

  • Level One accreditation for DFMEA, DRBFM, or PFMEA
  • Passed the following additional core tool training with certificate:
    -DFMEA
    -DRBFM
    -PFMEA
    -Machine Failure Modes and Effects Analysis (MFMEA)
    -Design for Manufacturing and Assembly (DFM/A)
    -Poka-Yoke
    -List the interactions between the product and process risk analysis processes
  • Led at least two Approved projects (DFMEA, DRBFM, or PFMEA)
  • Proven facilitation skills in the selected process
  • Signed approval by management on at least two completed projects

Level Three: Proficiency

Upon completion of Level Three accreditation the participant will have completed the following:

  • Level Two accreditation for DFMEA, DRBFM, or PFMEA
  • Passed the following additional core tool training with certificate:
    -APQP / PPAP
    -Fault Tree Analysis (FTA)
    -Functional Safety
    -Root Cause Analysis (RCA-8D)
    -Quality System Basics (QSB) -Measurement System Analysis (MSA)
    -Statistical Process Control (SPC) -List the interactions between the product and process risk analysis processes
  • Led the completion of at least four Approved projects (DFMEA, DRBFM, or PFMEA)
  • Mentored at least two people through Level Two accreditation
  • Trained at least two groups in risk analysis (DFMEA, DRBFM, or PFMEA) training

Level Four: Expert

Upon completion of Level Four accreditation the participant will have completed the following:

  • Level Three accreditation for DFMEA, DRBFM, or PFMEA
  • Passed the following additional core tool training with certificate for those selecting DFMEA or DRBFM:
    -Reliability Engineering
    -Design of Experiments (DOE)
    -Lean Six Sigma (DMAIC)
    -List the interactions between the product and process risk analysis processes
  • Led the completion of at least six Approved projects (DFMEA, DRBFM, or PFMEA)
  • Mentored at least four people through Level Two accreditation
  • Mentored at least two people through Level Three accreditation
  • Signed approval by management on the six completed projects
  • Conducted risk analysis training for at least four groups in both design (DFMEA or DRBFM) and manufacturing (PFMEA)
