COURSE DESCRIPTION

RCA (Root Cause Analysis) using the 3-Legged-5-Why technique provides a methodical approach to use in the development and post-product phases to assess issues in design, development, and production.
The RCA course presents industry best practices for remediation of problems, and provides a step-by-step method to minimize re-occurrence of these problems in your organization. This course is intended for product designers, developers, and supplier quality engineers. This is a foundation training module and does not have required prerequisite training or experience.

COURSE OBJECTIVES

This course is intended to provide an overview on RCA, using the 3-Legged 5-Why approach. At the end of this course you should be able to:

⦁ Define the 5-Why analysis
⦁ Define the “3 Legs” interaction within the 5-Why analysis
⦁ Define the steps to solve a problem
⦁ Define the problem and provide a clear and accurate problem statement
⦁ Define the problem as the customer sees it
⦁ Define traceability to the people doing the work
⦁ Define detection processes and methods controllable in your plant

COURSE OUTLINE

RCA is a structured method that visually displays the logical steps to efficient problem-solving. RCA will support your organization’s Functional Safety systems, and ensure sensitive sub-systems have oversight and proper “due care”.

This course will cover the following RCA items:

⦁ RCA Basics using the 3-Legged 5-Why methodology
⦁ Benefits of RCA in your development stages before production
⦁ Use the 3 “Legs” in the 5-Why methodology to ensure problems are surfaced before production and resolved to minimized re-occurrence:
⦁ Specific
⦁ Detection
⦁ Systemic
⦁ Use the 3-Legged 5-Why methodology with other problem-solving tools
⦁ Using 3-Legged 5-Why for internal plant issues such as FTQ, scrap reduction, and internal audit NCR’s

At the end of the training the participant will be required to take quizzes that test proficiency and complete a course survey based on the course material and the instructor. ReliaTrain uses the results of the quizzes and surveys to update the training to provide the best experience to participants.

COURSE PREREQUISITES

This is a Level One foundation training module and does not have required prerequisite training or experience. However, this course is a Prerequisite for higher level (Competency, Proficient, and Expert) FMEA Certification.

This course is intended for management and engineers in areas of design, manufacturing, assembly, quality, service, packaging, material handling, FMEA facilitators, and supplier quality (Including Suppliers).
